The Home Care Consultant is a professional responsible for facilitating and collaborating in the hospital setting to effectively and efficiently transition patients to home care and the ambulatory space.
Position Overview:
As a Care at Home Home Care Consultant, you will operate within the hospital, closely collaborating with key partners such as the Emergency Department (ED) and Inpatient care coordinators. Your primary focus will center on face to face UPH key partner, patient and family education, ensuring seamless transitions from hospitalized care to home-based services. In this pivotal role, you will coordinate services for patients, collaborate with referral sources, and provide essential support to our community. Additionally, you will assess individual patient needs and align them with the comprehensive services offered by UnityPoint Care at Home, utilizing available tools. You will leverage the RN scope for screening and triaging patients who qualify for services such as Hospital Care at Home & Primary Care at Home. Education: Graduate of State Board approved program for Registered Nurses and valid license as a Registered Nurse. BSN preferred.
Experience: 2 years of Hospice experience. Previous sales/marketing expereince, preferred.
License(s)/Certification(s): Person Centered Care (PCC) course completion within first 12 months of hire and annual completion of competency validation activities.
Knowledge/Skills/Abilities:
Strong interpersonal skills and customer service skills.
Strong problem solving skills.
Basic knowledge of computer applications (i.e. Word, Excel, EPIC) and operations.
Ability to understand and apply guidelines, policies and procedures.
Demonstrates the ability to interact effectively with physicians, health care team members, third party payers, and members of their support services.
Demonstrates flexibility, priority setting, problem solving, conflict resolution, negotiating and networking skills, decision-making, work delegation and organization, and excellent verbal/written communication skills.
