Postion Title: Supervisor Community Based Services

Area of Interest: Aging Services

FTE/Hours per week: 1.0, 40 hours per week

Department: Aging Services-Cedar Rapids Milestones

Shift: Monday-Friday 8:00 am to 4:30 pm

POSITION SUMMARY

The Supervisor Community Based Services will be responsible for the day-to-day operations of Aging Services' community based services for the respective service area and ensuring seamless team management and development, program delivery, and quality improvement and evaluation. The Supervisor Community Based Services will have a community presence and maintain a commitment to the agency and the essence of aging and community-based services.

Responsibilities

Essential Functions/Responsibilities:

Leadership Responsibilities

* Cultivate existing relationships with community agencies with the goal of ensuring resources, and access to aging services.

* Participate in the development and implementation of best practices and identify best practices associated with providing aging services.

* Participate in program development of current and future programs to ensure quality services to clients served.

* Participate in the development and implementation of orientation programs for staff and provide ongoing mentoring of staff.

* Community leadership and community collaboration with community agencies at local and state levels.

* Provide direct supervision and evaluation to direct service staff.

Organizational Relationships

* Report to Director of Aging Services.

* Responsible for supervision of program staff in the service area.

* Liaison between Aging Services and external contacts such as referring agencies, hospitals, service funders, licensing/accreditation entities.

* Coordinates the recruitment, screening, and training of volunteers for community programs

* Assures compliance with reporting requirements for community programs

* Performance evaluated by Director of Aging Services

Team Management and Development

* Participate in the development and implementation of training programs for the professional development needs of all staff.

* Work with staff to develop objective performance measurements across all sites, to ensure consistent, high-quality evaluation and goal setting for all employees.

* Participate in the recruitment, hiring, and evaluation of community based services staff.

* Participates in the development of quality improvement measures for aging services.

* Participates on internal quality improvement and Medicaid Integrity program committees to ensure implementation of and oversight of policies and procedures related to corporate compliance and Medicaid integrity compliance, where applicable.

* Provides oversight, direction and technical assistance regarding program policies, procedures, standards, rules, regulations and plans to ensure quality services to the clients served.

* Coordinates compliance training and oversees Quality Assurance activities as assigned by the Director of Aging Services.

* Develop, implement, monitor and evaluate program delivery improvement.

* Develop, implement, monitor and revise policies and procedures related to community based programs as required by governing bodies.

* Provide initial and ongoing training on new policies to all staff.

* Coordinates with the Director and direct service staff to ensure stakeholders involvement in program policy development.

* Other duties as assigned.

* Is knowledgeable of department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.

Qualifications

QUALIFICATIONS:

Minimum Requirements

Education:

Humans Services related bachelor's degree

Experience:

At least five years of experience with three of those in a team management supervisory role preferred.

License(s)/Certification(s):

Valid driver's license when driving any vehicle for work-related reasons.

Knowledge/Skills/Abilities:

* Broad knowledge of licensure, accreditation, governing body standards for community based programs.

* Strong leadership skills and experience in facilitating interdisciplinary collaboration.

* Strong organizational skills.

* Excellent verbal and written communication skills with exceptional attention to details.

Other:

* Demonstrated success developing and evaluating program models, and selecting and successfully operationalizing innovative programs.

* Proficient in using technology as a management reporting tool and experience working with information technology staff to develop and implement program evaluation systems.

* Strong project management skills managing complex, multifaceted projects resulting in measurable successes and program growth.

* Strong understanding of human resources, employee performance improvement plans, and corrective action policies.
